These students are required to provide proof of a physical examination and complete immunizations.

You will be required to submit proof of a dental examination.

Vision Examination for: Kindergarten
You will be required to submit proof of a vision examination by October 15th.

Students enrolling in an Illinois school for the first time must provide proof of a physical examination with the appropriate immunizations, a dental examination, and a vision examination. Please consult the school nurse for additional information

The Immunization Report
Each year our school files an immunization report with the ISBE (Illinois State Board of Education). This report will be available to the public on December 1st of each year, by contacting the School Nurse at (708) 458-1152 extension 321.
